This Change/Cancel form is used for both the Club Calendar and the Pavilion Calendar.               

If you wish to cancel/change your event only on the Pavilion Calendar (your location has              

changed), please check the Pavilion Calendar box at the bottom of the form.  If the event is          

to be canceled or changed on both calendars, please check both boxes at the bottom of the form.
