1. Visit 15 different MVDC member clubs that you are not a member of.
2. You cannot get this form signed during a raid/retrieve dance.
3. This form must be signed during the dance by the Raid Chairman or President of the club you are visiting.
4. Turn completed form into your club Banner Raid person to submit to the MVDC.

This award program is run by the Ohio Corporation of Dance Clubs, of which MVDC is a member. As such, dancers who are members of MVDC clubs are eligible to participate. This program is presented here as a courtesy.

2. Rules
1. Dancers must attend a club dance or festival in a listed area to receive the area stone.
2. Clubs visited must be members of their area federation/council.
3. After you attend a dance outside your own area, you can order your badge and first area stone from your State Corp. representative.
4. To receive your own area stone you must dance at a club you are NOT a member of or are NOT raiding.
5. Attendance at a state convention or State Fair Day may be substituted for one area.
6. An officer of the club/convention/State Fair Day must sign your form at the dance.
7. Each time you have a signature you may get a badge or area stone from your state representative.
8. Form is not transferable.
